Canada Day Weekend
Please be aware that our Doctors will not be in office on
Monday, June 30 and Tuesday, July 1.
Reception will be available on Monday, June 30 from 8:45am to 12 noon only.
The office will be closed for Canada Day on July 1, 2025.
We will be back to regular hours on Wednesday, July 2, 2025.
Uninsured Services
Uninsured services are those services provided to patients that are not covered by the Ontario Health Insurance Plan (OHIP) as they are not considered to be medically necessary. It is the patient's responsibility to pay for these uninsured services before seeing the doctor.
Payment for all uninsured services is payable at time of completion/pick up and is required in CASH only. Forms/notes are not released without payment. Receipts will be given for all payments.
Here is an example of common services:
Accessibility Parking Permit Application - no charge
Disability Tax Credit Application - $90.00
Sick/Massage Therapy/Orthotics Notes (without appointment) - $25.00
Functional Abilities Form - $50.00
Ministry of Transportation Driver's Medical - $180.00
Marine Medical - $250.00
Transfer/Copy of chart (printed) - $35.00 for 1st 20 pages, $0.30 per page after
Transfer/Copy of chart (USB) - $35.00
If you are requiring any forms to be completed, please make sure that your portion and signature has been completed prior to bringing to doctor.
